This is one of the puzzles that needs to be completed in Chapter 1 in order to remove another piece of the mirror by the front door. It can found in the office cabinet. But first you have to get it open. If you complete the 5 Bottle Puzzle on the first try, you can get the Simple Alchemy Trophy as well.
Note that in order to complete this puzzle you’ll need the bottle found in the music box.

To open the cabinet, go to the Apothecary and look in the cauldron. Take that star and put it in the cabinet door.
Now go to the desk in the office. In the top drawer is a note that you’ll need.
Use the note to determine the order of the bottles. If you get the order right on the first try, you’ll get the Simple Alchemy Trophy. Read the note carefully. Note the words “RIGHT BEFORE” on the second line. So instead of eyes, nose, ears, hands, tongue, you need to switch nose and ears.
