In the forest chapter, in the starting Full Moon settings, to the right of the starting mirror is a tree with a door in it. Go in there for the Eyeball puzzle. Completing three different labyrinths will reward you with a creepy bait, or a jar full of eyeballs.

The eyeball will move until it is stopped by something. It’s not possible to stop it early.
